To be a cathedral, a church must be the seat of a bishop, and the bishop of Rome—whom we know mostly as “the pope”—technically sits at a cathedral about three miles southeast of the Vatican called the Archbasilica of Saint John Lateran, thereby ranking it above every other Catholic church in the world.
Basilica, meanwhile, can signify pretty much whatever the pope wants it to.
